BayCare Health Systems is currently seeking an experienced Case Manager to join our outstanding and compassionate team in Hernando County! This is a field-based position that works throughout all of Hernando County. This position has an on-call component for after-hours and weekends.

Location: Brooksville, Hernando County

Status: Full-Time, Exempt: No

Shift Hours: 8:00 AM - 4:30 PM

Shift: Days

Weekend Work: Occasional

On Call: Yes

The Case Manager is responsible for:

• Providing linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.

• Coordinating the service delivery system for patients.

• Providing residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.

• Performing other duties as assigned.

Required Education:

• Bachelor's in Related Field, Human Services Preferred

Experience:

• 1 year in Mental Health required

Required Certification/License:

• BLS (Basic Life Support)

• Driver's License
